DD-5
The DD-5 is a form used by the New York City Police Department (NYPD) to make a report of an ongoing investigation. It is also known as the complaint follow-up form. This form is used to document further work done on an investigation. In addition, the term “DD-5” can also refer to the Case Management System, which is a digital notepad that detectives use for complaint follow-ups.
On February 21, 2025, Luigi Mangione’s lawyer KFA told the press that she hasn’t even received any of the DD5s pertaining to his case.
There have been instances where NYPD officers were accused of falsifying or doctoring DD-5 forms, used to document ongoing investigations.
One notable case is Rodriguez v. City of New York, where several allegations of misconduct involving DD-5 forms were made.

The name DD5 comes from old slang for “document detectives file.”
